Parent or Guardian Permission

Please ask your parent or guardian to complete this section.

My child named above has my permission to write for the Carroll County Public Library Teen Blog. I understand that he/she will be responsible for at least one blog post per month. The content of the blog will be reviewed by moderators before it is posted. All comments submitted by teens or the public will be reviewed before they are posted. Contributing writers may publish their first names and current grade level with each blog submission. Service hours for blogging will be awarded in May and November. If a student needs service hours prior to the scheduled times, please make arrangements with the blog coordinator. In general, a student will receive 3 student service hours for each blog post. If a student feels he/she should receive more hours for a post, he or she must contact the blog coordinator at the library to discuss the situation.
